Talk to an insurance broker that handles individual health policies. Beware of “internet” offers.

If you want specific companies and suggestions, you need to provide a lot more information. You did not even tell us where you live. A company that offers very good insurance at a good rate where I live might not even write where you are.
It is not likely that you will find a carrier that will write a plan for you that will cost less than physician visits and medicines. They are taking on so much more responsibility for your health that the premium will more than likely be more than your current out of pocket expenses for physicians and medicine.

The fact that you are already taking medications indicate that you have a pre-existing health condition. Depending upon the this unspecified health condition you may not be able to qualify for insurance through a private commercial carrier. They may not write the insurance at all or they may write the insurance but exclude coverage for your health condition.
In many Says Blue Cross Blue Shield will accept you without regard to your health status.
Check with an agent. He/she know the markets available in your Say and can propose plans that take into account your health status and your limited budget (but be prepared health insurance is not inexpensive).
